the kind of day care license or certification you will need
street address for the property or lot number from a tax plate (map) available at http://gis.mahoningcountyoh.gov, the County Courthouse, Sanitary Engineer, Planning Commission, District Board of Health, or zoning department
any utility easements or restrictions (call 1-800-362-2764) if you plan to build

Is the property zoned for operating the type of day care you plan to offer?

if you are constructing or renovating a home or building to be used for day care and sewer IS NOT available OR you need a food service license

Information you should have before you call:
the type of day care license or certification you will need
a tax plate (map) of the property
a plan of where the home or building will be located on the property and the location of any wells, easements, outbuildings or ponds

Questions to ask:
If you plan to operate a LICENSED DAY-CARE CENTER, is the property suitable for a commercial sewage system? (request a site evaluation)
If you plan to operate a FAMILY DAY-CARE HOME, is the property suitable for a household sewage system? (request a site evaluation)
If you plan to operate a LICENSED DAY-CARE CENTER, request a food service plan review application and information packet.

BEFORE YOU CONSTRUCT OR RENOVATE A HOME OR OTHER BUILDING FOR DAY CARE: The Permitting and Licensing Stage

Obtain a sewer tap permit from the County Sanitary Engineer, city, or village if sanitary sewer is available.
Obtain a zoning permit from the local zoning department

What you need to apply
a tax plate (map) from http://gis.mahoningcountyoh.gov, the County Courthouse, Sanitary Engineer, Planning Commission, District Board of Health, or zoning department
a site plan, building floor plan and exterior building elevation plan
an excavator bond (in some townships and villages)
a driveway or curb cut permit from the township, village, city, county or state (see list)
If you are constructing a LICENSED DAY-CARE CENTER, two sets of storm water retention plans
Obtain a footer drain and downspout permit from the District Board of Health
Obtain building and heating, ventilation, and air conditioning (HVAC) permits from the:

What you need to apply
three sets of construction, building, heating, plumbing and electrical plans for areas to be used for child care (the Building Inspection Department has a list of licensed heating and air conditioning contractors)
a zoning permit from the zoning department
plumbing permits from the District Board of Health
a downspout and footer drain permit from the District Board of Health
a sewer tap permit if sewer is available
for a FAMILY DAY-CARE HOME, a septic permit from the District Board of Health if sewer is NOT available
for a LICENSED CHILD DAY-CARE CENTER, a commercial sewage permit from the Ohio Environmental Protection Agency if sewer is NOT available
proof of electrical inspection from the Central Electric Bureau, 330-744-5238
the property address, parcel number, or lot number
Obtain plumbing permits from the District Board of Health
What you need to apply
a building permit from the Mahoning County Building Inspection Department or City of Campbell
contact the District Board of Health for a current listing of plumbers registered by the District Board of Health.
Obtain an electrical permit from Central Electric Bureau, 330-744-5238.

If you plan to operate a FAMILY DAY-CARE HOME, obtain a septic permit from the District Board of Health if sewer IS NOT available and the property is suitable for a household sewage treatment system.
Septic permit requirements
a septic system design from an installer registered by the District Board of Health (contact the District Board of Health for a current listing)
the address or parcel number of the property
a copy of the recorded deed or Mylar for the property
a site inspection from the District Board of Health to verify that areas on the property reserved for septic system components are fenced off
a building floor plan or blueprint
If you plan to operate a LICENSED DAY-CARE CENTER, obtain a commercial sewage permit from the Ohio Environmental Protection Agency if sewer is NOT available and the property is suitable for a commercial sewage system (Request a Site Evaluation)
Pay a water tap-in fee to the County Sanitary Engineer, city or village, or Consumers Ohio Water if city water is available
If city water is NOT available, choose a licensed well driller to obtain a well permit for you from the District Board of Health or Ohio Environmental Protection Agency
Before you choose a well driller
contact the District Board of Health for a current listing of licensed well drillers.
