What Comes Next? How do we implement this community health plan? Healthy Valley 2000 founding institutions realize that the same principles that brought them together to plan for healthier communities must keep them working together to assure that Mahoning County reaches the goals of this community health plan. A new charter for this collaboration, known as the Healthy Valley Alliance, has been adopted.
The Alliance is . . .
A partnership of organizations from the public, private and not-for-profit sectors collaborating to focus attention and resources on health problems identified as priorities through the Healthy Valley 2000 community health assessment and planning process.
Our Vision is of . . .
Healthy people and healthy communities in the Mahoning Valley.
Our mission is . . .
To promote the initiation, development and sustainability of initiatives that result in measurable improvements in the priority areas identified by the Commission for Community Health and future community health assessment and planning efforts. The Alliance will encourage collaborative action and efficient use of resources from multiple sectors and community systems in the Mahoning Valley.
Our Goals are . . .
- To promote coordination of community efforts to address the five priority health problems in Mahoning County identified by the Commission for Community Health in 1998:
Exercise for Wellness
Health Screenings
Substance Use Among Youth Under 18
Unintended Pregnancies
Violent Death- To embrace a broader definition of community that goes beyond traditional geographic and governmental boundaries, to include other counties in the Mahoning Valley.
- To communicate the vision, mission, and goals of the Healthy Valley Alliance to diverse community systems.
- To explore and implement strategies where collaboration achieves greater results than independent action. This includes linking the many initiatives geared toward promoting healthy communities.
- To relate the Healthy Valley Alliance agenda to the missions of organizations whose goals and initiatives improve the quality of life of Mahoning Valley communities and their residents. This includes working toward the achievement of the Healthy People 2000/2010 Health Promotion and Disease Prevention objectives.
- To create community indicators and systems that measure the progress of efforts that address health priorities identified by the Commission for Community Health.
Based on the charter of the Coalition for Healthier Cities and Communities, 1998.
