Located at St Paul the Apostle Catholic Church SR 170, (10143 Main Street) New Middletown, OH You child, ages 6 weeks thru 18 years, can receive immunizations and/or a physical exam.
The cost of a vaccine is $14.65 per shot. All childhood vaccines are availalbe including DTaP, Tdap, Hib, IPV, Hepatitis A, Hepatitis B, HPV, MMR, Varicella, Prevnar. The cost of a physical is based on a sliding fee scale and is performed by a pediatrician. We accept Healthy Start, Medical Mutual, Anthem, and Medicaid. No one will be refused services because of inability to pay.
